TEHRAN, May 28 (MNA) – Despite the fact that Saudi Arabia is among world’s wealthiest countries; poverty is an undeniable phenomenon in Saudi society.
While Saudi Arabia has wealthiest families, reports show 20 per cent of the population are under poverty line. Few hundred yards beyond the luxurious shopping malls and recreation centers of Riyadh, the beggars are searching for food, and a few miles farther, residents in slum houses in poor neighborhoods of southern Riyadh grapple with poverty to live.

Although Saudi government admits that poverty was a giant to fight with, the realities of the kingdom and problems face poor Saudi citizens had gone unnoticed by many. Exploring these terrains has proved difficult for foreign reporters, and that would be impossible except for the influence of famous individuals, who threaten their lives by walking on the same terrain.

Saudi Arabian government well knows the plights of the poor, but it has done little in providing help to assuage the effect of poverty. Much of the helps provided have been by third party private charity organizations.

Meanwhile, Saudi princes such as Walid bin Talal, one of the wealthiest and greatest investors, live a life of luxury.

Walid bin Talal, a famous Saudi prince and investor, has purchased an Airbus A380 jumbo jet for Saudi Rials 2b ($ 500m).

Deaths, lies and the NHS: Shocking new healthcare scandals emerge in UK

 

A British health manager warned the boss of the NHS four years ago that his hospital was a threat to patients’ safety. A nationwide investigation into avoidable hospital deaths has found that such warnings from doctors were often silenced or ignored.

Gary Walker, the former chief of United Lincolnshire Hospitals Trust, was silenced in 2010 under a £500,000 ‘super gag’ agreement; he is now risking legal action by the NHS by speaking out. Walker reportedly received the payment last year; his former employer now faces a major investigation over its unusually high death rates, in the midst of the Stafford inquiry fallout.

Walker said that the chief of the NHS chief was “not interested in patient safety,” and called on him to resign to end the“culture of fear” he had created in the NHS, Britain’s Daily Mail reported.

Walker spoke to the BBC in an exclusive interview for its ‘Today’ show on Radio 4, in which he explained that his hands had been tied: “I was in danger of losing my house – I have children to support. And one thing you must remember that if you’re attacking the very top of the NHS the sanctions are pretty dramatic.”

On Tuesday, shortly after learning of his plans to appear on national radio, the NHS wrote a letter to Walker reminding that, “If you have provided an interview or should this interview proceed you will be in clear breach of the agreement and as a result the Trust would be entitled to recover from you the payments made under the agreement.”

The letter reiterated that he was legally obliged not to inform anyone, besides family, of the terms of the gag agreement. The letter was released by the Lincolnshire Independents, a minor British political party, alongside other correspondences. The party said it was “shocked at the documents we have uncovered.”

Walker was fired in 2010, allegedly for using profanity during meetings. He and his supporters claimed that he was forced out for whistleblowing.

Leaked Letters reveal systemic neglect

New leaked letters have shown that doctor concerns over the Lincolnshire hospital were constantly ignored, despite that medical staff often challenged management about policies that threatened patient safety. The number of ‘excess’ deaths at Lincolnshire Hospital stood at 677 between 2009 and 2012, according to David Bowles, the former chair of United Lincolnshire Hospitals Trust. Bowles said he quit over such dangerous target-setting.

In one letter, a doctor described a complex 10-hour medical procedure as a “major operation with major risks attached.” The patient’s operation was then postponed, because there were no Intensive Care Unit beds available at Lincolnshire Hospital in central England.

Another letter outlined the serious threat to the public caused by excessive targets, as well as intimidation from above:“I must make you aware of my concerns about the balance between patient safety and targets and inform you that in my view the current bullish and sometimes ruthless pressure from above on the management team in my Directorate is unfair and unacceptable,” a clinical director told Paul Richardson, NHS Trust Chair of United Lincolnshire Hospitals.

One more anonymous doctor begins, “I am writing in the immediate aftermath of today’s tragic death of an otherwise well patient,” adding that ward staffing levels were inadequate for postoperative care, and that the “enormous pressure” exerted by target requirements resulted in ad-hoc arrangements for short notice surgery.

Lincolnshire Hospital is one of nine others that over the last week were slated to be investigated for unusually high death rates in the wake of the Stafford inquiry results. The Stafford inquiry was part of a long-term investigation into the hospital’s filthy wards, unnecessary deaths and understaffing.

Stafford Inquiry

The revelation comes shortly after inquiries into some 1,000 ‘avoidable’ UK hospital deaths in Stafford, central England, which concluded earlier this month that “corporate self-interest and cost-control” were to blame for the wider-scale NHS problems that allowed the deaths to happen.

Inquiry head Robert Francis, QC, urged for greater protection for NHS whistleblowers, as it emerged that many who try to speak out against hospital management are silenced. Many complaints were voiced about standards of patient care at the central England hospital, but they either went ignored or were given no adequate response, despite approximately 1,000 deaths.

Harrowing accounts of poor standards of care and hygiene and patients dying in undignified circumstances plagued the institution; further reports emerged of patient neglect and the failure to supply basic provisions.

One patient’s relative reported that health and safety rules prevented drinks from being left out at night, leading thirsty patients to drink water out of a flower vase. Relatives of two other patients reported that their family members had been dropped.  

Others fell victim to deadly infections rampant in the filthy wards, such as the superbugs C. dificile and MRSA. One relative said that her mother was so badly infected with C. dificile that she had to be buried in a sealed body bag.

Whistleblowers silenced

There have been serious attempts to discredit, bully, or fire NHS whistleblowers, which resulted in a media scandal last August.

Kay Sheldon, a board member of a UK healthcare watchdog, raised concerns that poor leadership and performance were a threat to public safety. She was diagnosed as possibly suffering from schizophrenia by an external assessor in a clear attempt to discredit her.

Many whistleblowers fear reprisal, as it there is a high possibility that they could be fired by those they raise concerns about. And in September, Labour MP Katy Clarke pointed out that lawyers representing the NHS were all too often silenced using gagging clauses when settling employment cases.

“I warned [David Nicholson, NHS boss] that Lincolnshire was going to become the next Mid Staffordshire. He didn’t investigate those concerns, and now look what’s happened,” Walker said.

“We cannot allow the disgraceful culture highlighted in the Mid-Staffordshire report to put Lincolnshire patients at risk,”said Chris Brewis, an independent politician on the Health Scrutiny Committee at Lincolnshire County Council.

The NHS and the government do not seem able to “learn from the old inquires that have been from 30 years ago”, David Lawrence from the London National Health Action Party told RT.

“The problem is that we don’t know yet how to avoid these [incidents] in the future and the government doesn’t know,”Lawrence added. “And that has got to be one of the main focuses of all our efforts – people who work in the NHS and all of us in the community to find out how to prevent the sorts of things going on. The pressures on the NHS have always been there. And I think over the last 20 years it got worse.”

As the Month Ramadan is approaching and will to start soon, I am posting this article is about the Islamic calendar month called Ramadan. Ramadan
 (Arabic: رمضان) is the ninth month of the Islamic calendar, and the month in which the Qura’n was revealed.

Fasting during the month of Ramadan is one of the Five Pillars of Islam. The month is spent by Muslimsfasting during the daylight hours from dawn to sunset. Muslims believe that the Qur’an was sent down to the lowest heaven during this month, thus being prepared for gradual revelation by the Angel Jibraeel(Gabriel) to the Islamic prophet Muhammad. Furthermore, Muhammad told his followers that the gates ofHeaven would be open all the month and the gates of Hell (Jahannum) would be closed. The first three days of the next month, Shawwal, is spent in celebrations and is observed as the “Festival of Breaking Fast” or `Eid ul-Fitr.

Timing;

The Islamic calendar is a lunar calendar, and months begin when the first crescent of a new moon is sighted. Since the Islamic lunar calendar year is 11 to 12 days shorter than the solar year and contains no intercalation, Ramadan migrates throughout the seasons. The Islamic day starts after sunset. Many Muslims insist on the local physical sighting of the moon to mark the beginning of Ramadan, but others use the calculated time of the new moon or the Saudi Arabian declaration to determine the start of the month. Since the new moon is not in the same state at the same time globally, the beginning and ending dates of Ramadan depend on what lunar sightings are received in each respective location. As a result, Ramadan dates vary in different countries, but usually only by a day. This is due to the cycle of the moon. When one country sees the moon, mainly Saudi Arabia, the moon travels the same path all year round and that same moon seen in the east is then seen traveling towards the west. All the countries around the world see the moon within a 24 hour period once spotted by one country in the east.

Each year, Ramadan begins about eleven days earlier than in the previous year. Astronomical projections that approximate the start of Ramadan are available. It takes about 33 years and 5 days for Ramadan to complete a twelve month move across the yearly calendar. As Ramadan March 28, 1990 to Ramadan March 22, 2023.

Events;

Ramadan is observed by Muslims during the entire lunar month by the same name. The month of religious observances consists of fasting and extra prayers.

10 Ramadan, death of Khadijah bint Khuwaylid – first wife of Muhammad

12 Ramadan, the Gospel (Injil) was bestowed on Jesus (Isa). (According to Shia Islam)

17 Ramadan, the Battle of Badr was won by the Muslims.

18 Ramadan, the Psalms (Zabur) were bestowed on David (Dawood). (According to Shia Islam)

19 Ramadan, Ali bin Abu Talib was struck on the head by a sword.

21 Ramadan, Ali bin Abu Talib died due to injuries he sustained by a sword.

Laylat al-Qadr is observed during one of the last ten odd numbered days of the month. Muslims believe that this night which is also known as “The Night of Destiny” is better than a thousand months. This is often interpreted as praying throughout this night is rewarded equally with praying for a thousand months (just over 83 years i.e. a lifetime). Many Muslims spend the entire night in prayer. 

In the Ottoman Empire, the sultan presented trays of baklava to the Janissaries in a ceremonial procession called the Baklava Alayı.

10 Ramadan, in 1973, Operation Badr took place, starting the Yom Kippur War.

Importance Of Women

The women should be good at raising her children.;

Because her children are the men and women of the future and the first one they are raised under his mother. So if the mother places emphasis on good manners, rather, she gives importance to ibaadah (worship) and good manners and good treatment towards the people and the children brought up by her hands are nurtured upon this, then they will have tremendous effect in the rectification of society.

So because of this, it is incumbent upon the women who has children and that she places great importance on their upbringing and the she seeks assistance, if she becomes incapable for any reason, from their father or whomsoever is responsible for their affairs if they have no father.

Either from their brothers or their uncles or their brothers from the same tribe or other than them. And it is not befitting for the women to submit to the state of affairs and say::

“The people are following this i can cannot change it.”

Because id we remain like this, submitting to the state of affairs, then the reflection will be be completed.

Very much like the relationships between men and women now. The conventions were different, but the essence of the relationships is the same. Some people are dominating and some are more retiring. Shakespeare shows us all kinds of couples. Couples which involve a strong man and a weaker woman are Othello and Desdemona, Demetrius and Helena, and Hamlet and Ophelia. Couples with a strong woman and a weak man include Helena and Bertram, Lady Macbeth and Macbeth, Goneril and Albany, Phoebe and Silvius, and Portia and Bassanio. Couples where both are strong include Beatrice and Benedick, Antony and Cleopatra and Petruchio and Katharine. Hero and Claudio both are weak. Shakespeare would not have shown us this broad array of different relationships had the reality of his world been as monochromatic as some would like us to believe.

As we follow our gut instincts sometimes, as if we struck with an idea about doing a repetitive rubbish in an entirely new way. We are not as restrained by our old habits now, as it empowering us to analyze our methods in a different light. I thought, we won’t ever know unless you give it a try. Even if I am not immediately successful, don’t give up; success may require multiple attempts before you get it just right. We all do our cockups time to time… We should remind ourselves that “patience is virtue”.

The phrase “patience is a virtue” is just a way of expressing the importance of being patient. A virtue is a trait or quality deemed to be morally excellent and thus is valued as a foundation of principle and good moral being. Patience can make us better people. The definition of the word is to tolerate delay implying self control and forbearance. When we say “patience is a virtue” we are exposing our moral values and expressing the importance of patience as a foundation of principle. So to answer the question patience is not a virtue until it truly becomes a virtue and knowing how diverse the spectrum of individualism is I’m sure patience is a vice (opposite of virtue) to some. Patience is a necessity for a happy existence and that is why the phrase is so often used. 

Came across an article and as I read which is written by Mamelody. And I asked myself the question, can men and women really just be friend? This is probably one of the most challenging aspects of human interaction. According to     survey conducted by the American Institute of Society Studies (AISS) it found that almost 98% of people who participated in that survey believe it is impossible for a man and woman to have a plutonic friendship without other factors being involved.

Take for instance my friend Jack Herness, a real estate broker. He has just been divorced after a three year marriage and he says that one of reasons to that was his friendship with women. “When I was dating my wife I made it clear to her that the majority of my friends were female. She accepted this in the beginning but gradually she could no longer control her jealousy to the point where she even fumed when I hugged my own sister,” he says. “I grew up in a house of women. I have four sisters, and I’m the last born and only son. My dad died when I was three so I was raised by my mother and sisters. All of my life my best friends have been girls.”

The fact is relationships between men and women are becoming complicated every day. It is normally impossible to imagine being friends with a member of the opposite without one factor in particular getting in the way – SEX!- Oscar Wilde puts this brilliantly from Lady Windermere’s Fan, which says, “Between men and women there is no friendship possible. There is passion, enmity, worship, love, but no friendship.”

Is culture reflecting reality?

Is it impossible for men to be friends with women, especially those they find attractive?

Statistics from the AISS also found that it is much easier for a woman to befriend a man with no sexual innuendo than it is for a man to be friends with a woman. Now being friends is not only about the sex, it also depends on other factors such as similarities in interests and hobbies, the level of communication and a deep understanding of one another.

“I do honestly agree that men and women cannot be friends, in the long run “says Carlos Hernandez, a relationship expert and coach.

“I met a wonderful lady about 27 years ago and we were great friends for like 13 years.  We did everything best friends do but deep down my heart I was attracted to her madly. So many times I wanted to just grab her and make passionate love to her but I respected our friendship so I controlled myself for 13 years. After we both suffered divorces we went out one day on our usual outings and we had a little too much to drink. We made hot passionate love and today she is my wonderful wife and I always regret the years we wasted as just friends.”

In reality, men and women need to be friends in order for anything else to take place. A relationship that is not based on friendship is doomed to fail.  That being said it is never easy to maintain a friendship with a member of the opposite sex but it can be done.   Most people who find themselves in situations like these where their partners are good friends with a member of the opposite sex, tend to develop insecurities which ultimately lead to the destruction of their relationships.

Mandy Goldberg, a senior nurse at St Jude’s hospital in Milwaukee begs to differ.

“I have a couple of very close relationships with women as well, though my closest platonic relationships are with men. My husband fully supports me and understands this and so he never sees the need to feel insecure,” she says.

“The trick to maintaining a healthy friendship with a member of the opposite sex is to be open about it to your partner, and make sure your partner is friends with them too, that way there will be no room for fidelity of any sort.”

Now what if you have been intimate with someone, will it be possible for you to just be friends? “Definitely not,” says Mike Dobson, an Airport Engineer.

“After intimacy the friendship line has been crossed. I did make the mistake of sleeping with my best friend who was girl and after that it was impossible to just be friends after I tasted the physical side.”

In conclusion, in the words of When Harry met Sally, Harry tells Sally “what I’m saying is –and this not a come-on in any way, shape or form – is that men and women can’t be friends because the sex part always gets in the way.”

Social factors that support women’s mental health

Women are encouraged to become competent at relationships, and as a result are better than men at seeking help and dealing with the causes of their distress. They also tend to be better able to give and receive help from each other.  Women who have strong family support, autonomy, and access to material resources that allow choice are better protected against developing mental health problems. 

Physical and mental health interactions

Physical and mental ill-health are linked in both men and women. People with chronic physical illnesses are at greater risk of developing mental health problems, particularly depression, while those with mental health problems are also more likely to have physical illnesses, such as heart or respiratory problems. 

MEDICALLY UNEXPLAINED SYMPTOMS

A range of physical problems are considered by doctors to be the physical manifestation of mental health problems, otherwise known as ‘conversion symptoms’ or ‘somatisation’. These include irritable bowel syndrome, fibromyalgia, and chronic pelvic pain. Studies show that medically unexplained symptoms such as these are two to three times more likely in women than in men.  Research shows that a high proportion of people with such problems have experienced trauma, abuse or violence.

UNRECOGNISED PHYSICAL ILL-HEALTH

An equally serious problem for women in mental distress is the lack of recognition for physical illnesses, because symptoms of physical illness may be wrongly seen as ‘imagined’ or psychosomatic. Research suggests that women with mental health problems are more likely than other groups to have physical complaints disregarded, and requests for services denied. Studies have found that women with mental health problems have significantly more undetected medical problems than men, and that women with bipolar disorder (manic depression) are three times more likely than men to have undiagnosed medical problems.

Treatment and services

Department of Health guidance on mainstreaming women’s mental health has pointed out that women ultimately want services to adopt a ‘whole person’ approach to their care, treatment and rehabilitation, to value their strengths and abilities, and to recognise their potential for recovery, in the context of holistic assessment and care planning.  Many areas of women’s mental health are now also covered by guidance from the National Institute for Health and Clinical Excellence (NICE).

ANXIETY AND DEPRESSION

Too often, a diagnosis of anxiety or depression leads to medication as the first or only treatment option. Women with these issues have repeatedly asked for better access to talking therapies, and for opportunities to learn new skills and coping strategies. These preferences are reflected in current NICE guidelines for the treatment of both anxiety and depression, which state that talking therapies are the most likely treatment to produce lasting benefits, especially when combined with other forms of social support and self-help. Government investment in the Improving Access To Psychological Therapies programme has increased the availability of short-term therapies such as cognitive behaviour therapy (CBT) via primary care. Pilot programmes show that over 60 per cent of those accessing this CBT are women, and that the intervention shows some positive results in reducing levels of depression and anxiety.

EATING DISORDERS

Talking treatments, including family therapy and CBT, are widely advised for women with eating disorders. NICE guidance on treatment for anorexia advises structured and symptom-focused inpatient admission as a last resort, with psychological treatment rather than behaviour modification. However, a lack of specialist eating disorder services across large areas of England and Wales means that local services often fall short of national standards.

SELF-HARM

For women who self-harm, NICE recommends full assessment of physical, psychological and social needs, by a professional with suitable training and in an atmosphere of respect and understanding. Treatment choices should include counselling and therapy. Women who have self-harmed severely should be referred to psychiatric services for further assessment, treatment and support, or taken into hospital in an emergency.

A study by an Asian Women’s group reported that a wide range of services is now on offer for young people across the UK, but there is still a need to evaluate which approaches really work, so that they can be widely adopted. Young women want a variety of options, including support for self-help techniques such as distraction – which includes a wide range of activities, from watching TV to gardening or flying a kite.

BORDERLINE PERSONALITY DISORDER

For women diagnosed with BPD, NICE recommends that a clearly structured, comprehensive multidisciplinary care plan that includes short- and long-term goals is agreed with the client. The care plan should include psychological treatments of at least three months’ duration, and plans to manage crises. No medications are specifically recommended for BPD. NICE and other agencies recommend a range of treatment options, including structured talking treatments, and medication for specific symptoms, such as transient psychotic episodes.

PERINATAL DISORDERS

Women clearly benefit from support before and after childbirth. One study shows that counselling for women who are depressed when pregnant may help to prevent problems for the family after the baby is born.  Other studies show that health visitors can provide valued listening and support to new mothers, and that individual, group and family support and counselling can help mothers to cope with postnatal depression and parenting problems. 

NICE has produced guidance on perinatal (ante- and postnatal) mental health, which recommends that health professionals look out for those most at risk of developing a mental disorder, and ask questions to detect problems, such as depression, as early as possible. This ensures that psychological treatment can begin as early as possible if needed, within one month of initial assessment, and no longer than three months afterwards.

NICE guidance recommends that clinical networks are developed in each region to ensure that services for mothers and infants are better coordinated, and that help is available quickly when needed.

TREATMENT FOR WOMEN DIAGNOSED WITH SEVERE MENTAL ILLNESSES SUCH AS SCHIZOPHRENIA

Experts have argued that gender-aware treatment for women with diagnoses such as schizophrenia should mean doctors taking more account of women’s practical and emotional needs and their social roles as partners, mothers, and professionals or employees. Women may need therapy and support focused on maintaining or re-establishing their roles, and perhaps will do better if they can receive help that does not separate them from their children, but helps them to cope better with parenting.

WOMEN WHO HAVE EXPERIENCED ABUSE OR VIOLENCE

Guidance has been developed to help health professionals work with women who may be experiencing mental disorders as a result of abuse, violence or trauma. The first step is to ask questions to find out about these experiences. Some studies show that, despite guidance, many staff do not do this well, perhaps because of a lack of training and experience.

The World Psychiatric Association recommends that women who have been sexually abused and those who have strong preferences for female healthcare staff should be accommodated whenever possible, and emphasises that the evaluation of mental health problems in women must consider the full context of their lives, as distress often has social origins. Diagnoses should not be stigmatising, and the role of violence and discrimination in the genesis of mental health problems in women requires special consideration.

SERVICES FOR WOMEN IN PRISON OR SECURE HOSPITALS

Women in prison who have mental disorders are rarely transferred to a hospital, but are treated by the NHS within the prison. Substance abuse is treated separately by Counselling, Assessment, Referral, Advice and Throughcare (CARAT). Offending behaviour is treated with CBT.

Treatment for women in secure psychiatric hospitals has been reformed by bringing all women in high secure units to Rampton, which provides a more specialised service. A new service development, Women’s Enhanced Medium Secure Services (WEMSS) aims to provide more local services, offering skilled nursing and psychological therapies, not just incarceration.


General issues relating to medication

SIDE EFFECTS AND INTERACTIONS

Side effects of psychiatric medication that women in particular may find distressing include weight gain and hair loss. In addition, medications can interact with each other in problematic ways, for instance, some medications interact with the oral contraceptive pill.

MOTHERHOOD AND MEDICATION

While medication for antenatal and postnatal depression is an option, NICE guidance  says that the risks of anti-depressant medication to the unborn child, or the infant through breast milk, should be explained, and talking treatments and self-help options should be explored.

The issue of medication is particularly important for women with schizophrenia or bipolar disorder who are mothers or who want to become mothers. Healthcare professionals need to work sensitively with women who are already on medication about whether or not it is safe and advisable to take a break from medication, taking into account the additional stresses of pregnancy and parenting, and the amount of support available. If this is considered possible, then experts advise tapering off the dose of medication in order to avoid the risk of damage to the developing foetus.

SAFETY ISSUES FOR WOMEN IN PSYCHIATRIC HOSPITALS

Sexual safety includes freedom from sexual harassment, exploitation, aggression and violence. Women are entitled to feel safe from physical harm or sexual harassment when in a mental health unit; there can be particular issues for women who have experienced sexual abuse or rape.

Separate sleeping, toilets and bathing accommodation for women in mental health units has been policy since 2000, and, according to the National Patient Safety Agency (NPSA), mental health units should be reconfigured to provide either a self-contained, women-only ward or solely single sex wards. Many, though not all, have done this. However, physical and sexual safety for women in mental health units is an ongoing issue, and the NPSA recommends better use of existing guidance and more training for staff to recognise and report incidents such as sexual harassment, taking into account the physical and psychological harm caused. 

Improving mental health policy and practice for women

Since the Department of HealthHealth’s report on women’s mental health, policies to improve treatment of women have become part of mainstream work. A number of NHS trusts have developed mental health strategies for women, and, in the light of gender equality duty, many are now further updating and reviewing their policies.

Current priority areas for improvement according to a recent national report are:

  • health and wellbeing
  • supporting women in their roles as mothers, carers, employees and students
  • safety and freedom from threat of abuse or violence
  • justice and fairness for women who come into contact with the criminal justice system
  • women’s participation, particularly empowering women from BME communities.

Many of the initiatives to improve services for women still have a long way to go, however. Women have been calling for more support for self-help and alternatives to medicalised treatments for many years, but in most cases these alternatives are not well supported financially, and are usually left to the voluntary sector to provide, and therefore not universally available.

Voluntary sector support

The voluntary sector provides many support services for women beyond what is available through statutory services, including information, advice and support and/or counselling, as well as drop-in facilities, befriending and advocacy. 

Some groups and organisations operating at local and national levels across the UK focus on the needs of women from particular BME groups. Many of these organisations are staffed by women who share the culture of their service users. Such groups can be useful for social opportunities or for specific services such as information, advice or counselling.

A range of organisations focusing on the needs of lesbians and bisexual women also operate locally and nationally. Most are run by lesbian, gay or bisexual people, and can provide social opportunities or specific services, such as information, advice and counselling. Counsellors or therapists who identify as lesbian or bisexual, or who work for organisations that are positive about LGB issues, can also be found via local organisations and internet as well.

Diagnoses Related To Reproductive Functions

 

Hormonal and reproductive changes can contribute to some mental health problems, and some diagnoses that women receive relate to aspects of their reproductive functions, including menstruation, pregnancy and childbirth, and menopause.

 

PREMENSTRUAL SYNDROME

Premenstrual hormonal changes have been linked to problems that can range from mild feelings of depression or irritation to, very rarely, premenstrual dysphoric disorder, characterised by anxiety, depression, insomnia, food cravings and feelings of being out of control. 

 

 

 

PERINATAL MENTAL DISORDERS

Women are particularly vulnerable to mental health problems in the time just before and after childbirth – the perinatal period. The main types of mental health problem that arise are:

  • depression in pregnancy
  • postnatal blues
  • postnatal depression
  • puerperal psychosis.

While pregnancy is widely believed to reduce depression, some studies have found that depression in pregnancy is more widespread than expected, especially in the third trimester. 

‘Postnatal blues’ is a normal emotional change that occurs in up to 50 per cent of women after childbirth,  and is thought to be linked to rapid changes in hormones. It is usually brief, although it is advised that women are monitored to ensure that this is not the start of postnatal depression.

Postnatal depression and puerperal psychosis are no different from depression or psychosis experienced at any other time. Some experts believe that giving birth acts as a major stress factor that can trigger the onset of these disorders in women who are predisposed to them. Postnatal depression can begin in the weeks following childbirth, or up to a year afterwards. The woman may feel low in mood and energy, worried about her child, and unable to sleep, or even have thoughts about abandoning or harming her child. 

While 13–15 per cent of women experience postnatal depression, puerperal psychosis is much rarer, affecting only about 1 in 1000. It is a serious mental illness that can develop in a woman who has recently given birth, usually with no obvious cause. The baby may be healthy and wanted, and the birth is not unusually complicated.  (See Mind’s booklet Understanding postnatal depression.)

For women who already have mental disorders, such as schizophrenia or bipolar disorder, pregnancy can bring additional stress. Some women with bipolar disorder may experience a worsening of their symptoms during pregnancy and after the birth.

Diagnoses Women Receive Less Frequently

 

SCHIZOPHRENIA AND BIPOLAR DISORDER

 

While there are no marked differences in the incidence of schizophrenia between the sexes, women tend to be older than men by three to six years when first diagnosed, often in their late 20s, and there is a further peak of onset in women after 45 years of age. There are also differences in the types of symptoms women experience and in rates of recovery. Researchers therefore recommend a gender-sensitive approach to the diagnosis and management of schizophrenia. 

The type of bipolar disorder experienced by women is usually characterised by mild ‘hypomanic’ episodes, but with a greater burden of depression; researchers also recommend a gender-sensitive approach.

 

 

SUBSTANCE DEPENDENCE

Substance (drug and alcohol) dependence is more common in men but the experience of substance dependence is often different for women. Traditionally, a greater social stigma is attached to women who are dependent on illegal drugs or alcohol, which can lead them to hide their problems, and choose not to access health or social care services. Women who are lone parents are particularly likely to hide their problems through fear that they may lose custody of their children. In these situations, substance dependence and co-existing mental health problems are likely to become more severe over time, with serious consequences for a woman’s health and well being. 

Women are more likely than men to become dependent on prescription medication, including antidepressants and tranquillisers (for anxiety or use as sleeping pills). This is partly because women are more frequently diagnosed with anxiety and depression and given medication to treat these conditions.
